Penis Enlargement or Phalloplasty
For centuries, penis size has been a source of serious concern and anxiety for the male population. Nevertheless, in most men, penis length is within normal limits. Concern about penis size can lead to low self-esteem, sexual dysfunction, and even psychiatric disorders. The term "penile dysmorphophobia" was introduced to describe the distorted perception a man may have of his penis size, even though it is of normal dimensions. Most patients requesting penis enlargement fall into exactly this category. However, the satisfaction rates of men after penis enlargement surgery are low. Mainly because the formation of even small hypertrophic scars and keloids (abnormal scars) of these men distorts the image that they would like for their mole.
For these reasons, Dr. Mertziotis and his team invented a new technique "without incisions" which has been applied for at least a decade and was published in an international urological journal in 2013 and was recognized worldwide as an innovative technique with very low complication rates and at the same time as the safest. In essence, with a "circumcision" i.e. an incision under and around the belly button which is not visible, it makes the penis elongate and increase the diameter of the penis.
Common questions and answers about penis enlargement
1. Q. Do men who want to enlarge their penis usually go to a urologist?
A. No. Most of the time they do not know that this procedure is performed by specialized urologists and usually turn to plastic surgeons.
2. Q. Can a man's feeling about the size of his penis, even if it is normal, affect his relationships with his sexual partner(s) and professional partners?
A. It is possible.

A. In relation to the feeling of penis inadequacy, i.e. the perception that a man has that his penis size is smaller than he would like, regardless of whether it is normal (called "penile dysmorphophobia"), the results of such a surgery will improve his sexual image and the image he has "as a man" as well as positively affect his relationships with his sexual partners, colleagues and associates.

Results — length, diameter and permanence
8. Q. What can be the increase in penis length?
A. This is a difficult question to answer, and absolute promises cannot be made since each case is different. In general, the increase ranges from 2–4 cm. The maximum length is achieved if the surgery is combined with the penis enlargement device.
9. Q. What can be the increase in penis diameter?
If you have already undergone a diameter increase with liposuction and fat injection the results vary. I do not use this technique.
With the technique of autologous fat grafts that I use, the diameter of the penis can be increased from 2–3 cm when the penis is in a relaxed phase, i.e. the diameter increases by about 30–40% and 10–20% when it is erect.
10. Q. How long after the operation can I have intercourse?
A. It is preferable to wait about 6 weeks before having vaginal penetration.
11. Q. Are the results of penile lengthening permanent?
The results of penis lengthening are permanent, especially when combined with a penis enlargement device and also gaining extra length over time and with the use of the device.
12. Q. Are the effects of increasing penis diameter permanent?
The effects of liposuction and fat injection are often not permanent. Usually within the first year more than 50% of the injected fat has been absorbed by the body and for this reason I no longer use this technique. In addition, asymmetry, hard lumps, and hardness of the penis can be observed even if it is in a relaxed phase. The autologous fat grafts technique I use offers excellent aesthetic results and seems to be more predictable and permanent in time.
13. Q. What are the success rates of these procedures?
A. It depends on what you consider success. There is a very high percentage of pleasure among my patients who undergo phalloplasty with my technique in terms of lengthening and increasing the diameter mainly in flaccid phase of the penis but also in erection. Almost all men who are informed of reasonable results and have reasonable expectations are completely happy with the operation. Before surgery it is very important to discuss the surgical technique, where the incisions will be made and what the postoperative course will be. When this is done then all men are happy with the result and consider the operation successful.
Non-surgical methods
14. Q. Is it possible to increase the size of my penis without surgery?
A. There are many undocumented non-surgical methods and devices that use weights or vacuum or vacuum devices. The medical community does not recommend these techniques since there have been no scientific studies to show how effective they are or what risks can be caused by their use. In fact, their advertising in our country is illegal and prosecuted, but in practice of course it does not happen. There is no proven non-surgical technique that increases penis diameter.
Risks and complications of surgery
15. Q. What are the risks and complications of surgery?
A. First of all, let us clarify that the vast majority of the operation requires only 1 day of hospitalization and lasts about 2 hours and 15 minutes on average.
The complications from the penis lengthening technique are very few and of minor severity. However, I will discuss with you possible risks related to:
- • a decrease in the angle of the penis upwards when it is erect
- • development of keloid (hypertrophic) scar at the incision site
- • wound suppuration
- • decrease in penile sensation
The complications from the technique of increasing the diameter of the penis are very low. More generally, the possible risks we may discuss are:
- • Resorption, shrinkage, or failure of the graft to survive less than 3%
- • Penile shortening <10%
- • Swelling, suppuration and skin loss of the penis (very rarely <0.5%)
